The ideal candidate will play a key role in advancing patient care by ensuring adherence to evidence-based guidelines and contributing to quality improvement efforts.
Key Responsibilities:
• Data Abstraction and Reporting:
- Perform detailed abstraction of clinical data from patient records for GWTG Stroke and Heart Failure registries.
- Ensure the timely and accurate submission of data to meet national and accreditation standards.
- Identify and resolve documentation in patient records and communicate the findings back to the client.
- Collaborate with multidisciplinary teams to identify and address gaps in documentation and care processes.
- Assist in the development and implementation of quality improvement projects focused on stroke and heart failure care.
- Generate reports and dashboards to communicate performance data to stakeholders and leadership.
- Provide education to clinical staff on core measure requirements and guidelines for stroke and heart failure management.
- Conduct feedback sessions with staff to address performance gaps and improve compliance.
- Work closely with quality, nursing, and clinical teams to ensure adherence to Joint Commission, CMS, and other regulatory requirements.
- Participate in meetings and team calls related to stroke and heart failure care improvement.
- Proficiency in electronic health record systems (e.g., EPIC, Cerner) and clinical data systems.
- Strong analytical skills with the ability to interpret and report complex data accurately.
- Excellent interpersonal and communication skills for engaging with multidisciplinary teams.
- Highly organized and detail-oriented, with the ability to manage multiple projects effectively.
• Licensure/Certifications:
- RN, RHIA, RHIT, or other relevant healthcare certification preferred.
- Certification in Stroke (SCRN) or Heart Failure (CHFN) is a plus.
- Minimum of 2 years of experience in clinical data abstraction or quality improvement.
- Hands-on experience with GWTG Stroke and/or Heart Failure registries is highly desirable.
